Seven observations are charted there. Two of them change what you do next. The overnight weight gain of 4 lb is roughly 2 liters of retained fluid, and the change from one pillow to three is new orthopnea. The other five are stable, reassuring, or irrelevant to the decision.

VITAL SIGNS — every recorded set with its time, plus the admission baseline. LABORATORY RESULTS — results with their reference column printed beside them. MEDICATION ADMINISTRATION RECORD — scheduled doses, given times, held doses, and every as-needed dose with its time.
The tab decoder
Vital signs answer is this client stable right now. Laboratory results answer what is the mechanism. The medication administration record answers what did we already give, and when. Nurses' notes answer what changed.
The medication record callout. Two things get missed there. The time of the last as-needed dose, which tells you whether a symptom is untreated or unresponsive. And a held dose with its reason, which is often the entire explanation for the finding you are looking at.
