Every unit on this page and on pages 70, 72, 73, and 75 has the same three parts. The key, restated in full. A line for every option that was not keyed. And one sentence naming the principle that would have produced the answer without knowing the topic.
Item 1 — Answer: 2
Why 2 is right: measuring and recording intake and output is a standardized task with a predictable outcome that does not require assessment, judgment, or teaching. The registered nurse remains responsible for interpreting the totals. Why 1 is wrong: the first ambulation after a joint replacement requires assessment of orthostatic tolerance, pain control, weight-bearing status, and the surgical site. First-time anything is nursing assessment wearing the costume of a task. Why 3 is wrong: teaching, including reinforcement of teaching about a high-alert medication, requires evaluation of understanding. Unlicensed personnel cannot evaluate. Why 4 is wrong: assessing an incision is assessment, and the word "assess" in an option is almost always the tell.
The rule: delegate the task, never the assessment, the teaching, or the judgment.
Item 2 — Answer: 1, 2, 4, 5
Why 1 is right: C. difficile is transmitted by the fecal-oral route on hands and surfaces, and contact precautions with a private room are the standard placement. Why 2 is right: the spore is not killed by alcohol, and soap and water works by physically removing spores from the skin. Why 4 is right: gown and gloves go on before entry, every entry, not only when contact is anticipated, because the environment itself is contaminated. Why 5 is right: routine disinfectants do not kill spores. A sporicidal agent, most commonly a bleach-based product, is required for the room and for shared equipment. Why 3 is wrong: alcohol-based hand rub is the default for most organisms and it does not kill C. difficile spores. Choosing it here is the single most common error on this item and the one with the clearest real-world consequence. Why 6 is wrong: negative pressure and airborne precautions are for organisms transmitted on droplet nuclei that stay suspended. This organism does not travel that way.
The rule: spores are not killed by alcohol.
Item 3 — Answer: 1
Why 1 is right: recombinant zoster vaccine is recommended for immunocompetent adults aged 50 years and older, as a two-dose series with the second dose two to six months after the first. This client is 52 and is due. Why 2 is wrong: 60 was the age band for an older live vaccine that is no longer the recommendation. This is the classic right-answer-wrong-age-band distractor. Why 3 is wrong: the tetanus, diphtheria, and pertussis booster interval is 10 years. At 4 years out, the client is not due. Why 4 is wrong: it is simply false, and it is the option chosen by a candidate who has decided that adult immunization only starts at Medicare age.
The rule: age sets the schedule.
Item 4 — Answer: 2
Why 2 is right: it names what is happening, "this is a lot to take in," and then opens the floor with a question she can answer any way she likes. It neither predicts her feelings nor closes the topic. Why 1 is wrong: "try not to worry" is false reassurance. It tells a woman with metastatic disease that her reaction is the problem, and it ends the conversation in eight words. Why 3 is wrong: a chaplain referral may be entirely appropriate later. Offered now, it hands her distress to someone else and gets the nurse out of the room. Why 4 is wrong: telling a client what most people feel assigns her an emotion she did not report and invites her to agree rather than to speak.
The rule: a feeling named is a conversation continued.
Item 5 — Answers, in row order: indicated, contraindicated, indicated, indicated, contraindicated, indicated
Why full upright positioning is indicated: 90 degrees uses gravity and gives the airway its best protection during the swallow. Why upright positioning is not contraindicated: there is no swallowing benefit to reclining, and reclining is how aspiration happens. Why thin liquids through a straw are contraindicated: thin liquids move fastest and are hardest to control, and a straw delivers a bolus to the back of the mouth before the swallow is organized. This is the trap, because a straw looks like an assistive device. Why thin liquids through a straw are not indicated: speed of intake is never the goal in dysphagia. Why the chin tuck is indicated: tucking the chin narrows the airway entrance and widens the space food travels through, which is the single most useful position change in dysphagia. Why the chin tuck is not contraindicated: it is a taught compensatory technique, not a restriction. Why oral care before and after meals is indicated: before, it clears colonized secretions that would otherwise be aspirated with the first bite; after, it clears pocketed residue that gets aspirated an hour later. Why oral care is not contraindicated: nothing about dysphagia argues against a clean mouth. Why conversation during the meal is contraindicated: talking and swallowing use the same structures in opposite configurations, and a client who is speaking has an open airway at exactly the wrong moment. Why conversation during the meal is not indicated: alertness is confirmed before the meal begins, not by interrupting it. Why remaining upright 30 to 60 minutes after eating is indicated: reflux and pocketed residue both cause delayed aspiration, and gravity is still the intervention. Why remaining upright is not contraindicated: there is no competing need that requires lying this client flat after a meal.
The rule: gravity and control beat speed in swallowing.
