Learn the Class, Derive the Drug

The wrong strategy

Memorizing individual drugs is the wrong strategy. Thousands of agents are marketed; about sixty classes are tested. Studying drug by drug is buying a lottery ticket. Studying class by class is buying the whole shelf, because every member of a class inherits the mechanism, and the mechanism is what the item is really asking about.

The decoder rule

Established stems are assigned, not accidental. Manufacturers must use the recognized stem when a generic name is created, which is exactly why it is trustworthy. An unfamiliar name ending in -pril is an angiotensin-converting enzyme inhibitor, and every prediction that follows from that class applies to it on first sight.

Where the rule breaks. Three honest exceptions the exam exploits. -mycin and -micin split across aminoglycosides and macrolides, which carry different toxicities. -azole covers the antifungals and also metronidazole, an antiprotozoal and antibacterial. Older agents such as furosemide, a loop diuretic, carry no usable stem and must be learned by class membership instead.
