BLOCK 1 · THE PIPELINE, IN ONE LINE

  1. Maternal oxygenation and blood pressure → prolonged deceleration, bradycardia.
  2. Uterine blood flow → late decelerations from tachysystole or hypotension.
  3. Placental exchange surface → late decelerations, minimal variability, abruption.
  4. Umbilical cord → variable decelerations, prolapse, prolonged deceleration.

☐ Category I: baseline 110 to 160, moderate variability, no late or variable decelerations. ☐ Category III: absent variability with recurrent lates, recurrent variables, or bradycardia; or sinusoidal. ☐ Category II: everything else.

BLOCK 3 · BLEEDING CARD Painless, bright red, soft uterus, fetus stable early = previa, and no cervical exam of any kind. Painful, dark or concealed, rigid tender uterus, fetal compromise early = abruption, and prepare for expedited birth. Postpartum causes: tone (massage the fundus), trauma (inspect and repair), tissue (evaluate for retained fragments), thrombin (correct coagulopathy). Empty the bladder before the uterotonic.

BLOCK 4 · THE MINUTES LIST crit

  • Cord prolapse → gloved hand lifts the presenting part off the cord and stays there.
  • Postpartum hemorrhage → massage the fundus, empty the bladder.
  • Uterine rupture → prepare for immediate laparotomy.
  • Shoulder dystocia → hyperflex the hips, suprapubic pressure. Never fundal pressure.
  • Amniotic fluid embolism → airway, circulation, volume, left uterine displacement.

BLOCK 5 · NEWBORN DISCHARGE TEACHING CHECKLIST ☐ Supine for every sleep, firm flat noninclined surface, nothing loose in the sleep space ☐ Room sharing without bed sharing ☐ Rear-facing car seat in the back seat, correctly installed before discharge ☐ Feeding at least 8 to 12 times in 24 hours; feed on cues, not on crying ☐ Expected voids and stool color transition by day of life ☐ Cord and skin care; keep the base clean and dry, report redness, drainage, or odor ☐ How to take an axillary temperature and when to report one ☐ Jaundice progresses head to toe; call if it spreads below the face ☐ Newborn metabolic screening drawn after adequate feeding, with the repeat plan ☐ Hearing screening completed and critical congenital heart disease pulse oximetry screening done at the recommended hour of life

  2. Define gradual and define abrupt, using the thirty-second rule. (20 seconds)
  3. Say the timing of each of the four decelerations against the contraction. (60 seconds)
  4. Recite the intrauterine resuscitation sequence, first step to last. (60 seconds)
  5. State the three parts of the Category III definition. (45 seconds)
  6. Separate previa from abruption in two sentences, then say what is forbidden. (45 seconds)
  7. Name the severe features of preeclampsia without looking. (60 seconds)
  8. Say the cardinal movements in order. (45 seconds)
  9. Give the first two actions in cord prolapse and the first two in postpartum hemorrhage. (45 seconds)
  10. Name the four newborn heat-loss routes and say why the newborn cannot shiver its way out. (45 seconds)
  11. Name the Apgar components and say what a score of 1 for color describes. (45 seconds)
  12. State the one jaundice rule that is never negotiable. (20 seconds)
