Unstable versus stable, unexpected versus expected

TWO SORTS, FOUR QUADRANTS
You can recite both distinctions and still lose the item, because they are two different axes and candidates collapse them into one. A patient can be unstable and completely expected. A patient can be stable and deeply wrong. The exam lives in that gap.

Why the order runs 1, 2, 3, 4
Unexpected instability wins because nothing about it has been anticipated. Nobody has written an order for it, nobody has decided how far it is allowed to go, and the finding itself may be the first hour of something that has no plan. Expected instability, by contrast, arrives with a plan already attached. The sickle cell patient is in severe pain because vaso-occlusive crisis produces severe pain, and there is an infusion running for exactly that reason.
Stable and unexpected comes third, not fourth, because a new calf finding after knee arthroplasty and bleeding gums on heparin are both early signals of things that become emergencies later in the shift. Stable and expected comes last and often needs nothing but a look.
Drill · place each patient in a quadrant
- Day one after transurethral prostate resection, continuous bladder irrigation running, drainage pink with occasional small clots.
- Fifteen minutes into a blood transfusion, reports flank pain, chills, and a sense of dread.
- Day two of acute pancreatitis, pain severe and unchanged on the ordered infusion, sitting forward to breathe.
- Exit site of a peripherally inserted central catheter is newly red and tender this morning.
- First night after open reduction of an ankle fracture, pain in the casted leg rising despite medication, toes tight and hard to move.
- Day one after lobectomy, water seal fluctuates with respiration, chest tube drainage decreasing.
Answers: 1 stable and expected. 2 unstable and unexpected. 3 unstable and expected. 4 stable and unexpected. 5 unstable and unexpected. 6 stable and expected.
Watch outtingling around the mouth plus a hoarse voice after thyroidectomy is an airway problem in waiting, not a nerve complaint.
Margin noteexpected is not the same as fine.
