Six Links, Six Cuts: The Chain as an Engineering Problem

The four immunities

  • Active natural. Antibody the host made after surviving the infection. Long-lived, has memory.
  • Active artificial. Antibody the host made after a vaccine. Long-lived, has memory, takes weeks to build.
  • Passive natural. Borrowed antibody crossing the placenta or arriving in breast milk. Immediate, fades in months.
  • Passive artificial. Borrowed antibody given as immune globulin or antitoxin. Immediate, no memory, fades.

Live attenuated vaccines are withheld in pregnancy and in significant immunosuppression. That single fact is why the nurse who is pregnant, or who has no evidence of immunity, is not assigned to the varicella or measles room. The staffing answer flows from the immunity rule, not from the PPE rule.

Where the chain defeats you

  • Colonization is not infection, and the colonized patient still transmits. A resistant organism growing in a wound with no fever and no systemic signs still requires contact precautions.
  • Spores survive alcohol. Clostridioides difficile and Bacillus anthracis require soap and water for the hands plus a sporicidal agent on surfaces. An alcohol-based rub is not a failure of technique here; it is the wrong tool.
  • Equipment is a reservoir. The shared stethoscope, the blood pressure cuff, the glucose meter, and the bedside commode are links in the chain and are dedicated or disinfected between patients.

Watch outAn alcohol-based hand rub does not kill spores. Choosing it after caring for a patient with C. difficile leaves the organism on your hands and moves it to the next room.
