Set A Rationales: Why the Almost-Right Answer Is Wrong

Item 1 — Answer: 2 Why 2 is right: mumps travels on large respiratory particles that arc and settle, so droplet precautions apply. A single room is preferred and the door may remain open. Why 1 is wrong: reserves airborne engineering for an organism that does not stay suspended. ROUTE Why 3 is wrong: a curtain is not spatial separation and does not satisfy droplet precautions. ROUTE Why 4 is wrong: positive pressure protects the patient from the room, the opposite need here. ROUTE The rule: Droplet needs distance and a mask; only airborne needs a closed negative-pressure door.

Item 2 — Answer: 1, 2, 3, 5 Why 1, 2, 3, 5 are right: hand hygiene, exposure-based PPE selection, respiratory hygiene at the point of entry, and a mask for injection into the epidural space are all named elements of standard precautions. Why 4 is wrong: a respirator is transmission-based practice, correct only for airborne precautions. SCOPE Why 6 is wrong: gown and gloves for every room is contact precaution practice applied universally. SCOPE The rule: Correct practice in the wrong context is still a wrong answer.

Item 3 — Answer: 2, 3, 1, 4, 5, 6 Why that order is right: gloves come off first, then the face shield by its headband, then the gown. The nurse then leaves the room and closes the door before removing the respirator, and performs hand hygiene last. Why gown-first orders are wrong: the gloves are the most contaminated item and removing them last spreads what they carry. SEQUENCE Why removing the respirator at step 3 or 4 is wrong: taking the respirator off inside the room exposes the nurse to suspended particles and undoes the entire encounter. SEQUENCE Why placing hand hygiene before the respirator is wrong: hand hygiene is the closing beat, performed after all PPE is off. SEQUENCE The rule: Doffing order and doffing location are one answer, not two. Watch out: This distractor describes an action that would actually expose the nurse. Removing a respirator inside an airborne isolation room is a real harm, not a bookkeeping error.

Item 4 — Answer: 3 Why 3 is right: antibiotic-associated diarrhea suggests a spore-forming organism, and spores survive alcohol. Soap and water physically removes them. Why 1 is wrong: alcohol does not kill spores at any contact time. AGENT Why 2 is wrong: adding a rub after washing does nothing the wash did not already do and teaches a false safety step. AGENT Why 4 is wrong: repeating an ineffective agent does not make it effective. AGENT The rule: Spores mean soap and water for hands and a sporicidal agent for surfaces.

Item 5 — Answer: 2 Why 2 is right: the outer border band of the drape is not sterile, and anything crossing it, including a sleeve cuff, contaminates the field. Why 1 is wrong: the center of the drape is the sterile zone and the forceps belong there. SCOPE Why 3 is wrong: the table surface outside the drape was never sterile and is not the break. SCOPE Why 4 is wrong: proximity to the body does not contaminate; contact and level do. SCOPE The rule: The border band is the boundary the exam tests, and a cuff counts as contact.

Item 6 — Answer: 2 Why 2 is right: measles staffing is decided by documented evidence of immunity, so the correct action reassigns the patient. Why 1 is wrong: PPE does not substitute for immunity in the staffing rule. SCOPE Why 3 is wrong: limiting exposure time is a radiation principle, not an airborne one. ROUTE Why 4 is wrong: a surgical mask is not respiratory protection against an airborne organism. ROUTE The rule: Some isolation questions have a staffing answer, not a PPE answer.

Item 7 — Answer: 1, 2, 4, 6 Why 1, 2, 4, 6 are right: level, strike-through, line of vision, and the when-in-doubt rule are all real field rules. Why 3 is wrong: the stated border width is not the figure in current sterile technique guidance; the distractor is built from a near miss. SCOPE Why 5 is wrong: sterile touches sterile only, and the outer wrapper is not sterile. SCOPE The rule: A plausible number is the easiest invention to accept; verify the figure rather than the feel.

Item 8 — Answer: 1 Why 1 is right: the patient wears a surgical mask in transport, and the transporter maintains the precaution set in use. Why 2 is wrong: patients never wear respirators in transport. SCOPE Why 3 is wrong: dropping the transporter's protection because the corridor feels open abandons the precaution mid-encounter. SEQUENCE Why 4 is wrong: transport does not suspend precautions. SCOPE The rule: Precautions travel with the patient.
