How the Body Holds the Numbers Steady

Three defenders, three clocks
- Buffers respond in seconds. Bicarbonate, hemoglobin, protein, and phosphate soak up hydrogen ions before anything else moves.
- The lungs respond in minutes by changing PaCO₂, normal 35 to 45 mm Hg. Blow off carbon dioxide and the pH rises. Retain it and the pH falls.
- The kidneys respond over hours to days by holding or dumping HCO₃⁻, normal 22 to 26 mEq/L. Slow, but the only defense that lasts.
- pH normal 7.35 to 7.45. The midpoint, 7.40, is the line compensation aims at and never crosses.
What moves together
- Potassium and hydrogen trade places across the cell membrane. Acidosis pushes potassium up. Alkalosis pulls potassium down.
- Calcium and phosphate move inversely. One rising predicts the other falling.
- Chloride shadows sodium and moves opposite bicarbonate.
- Albumin, normal 3.5 to 5.0 g/dL, carries calcium. A low albumin drags the total calcium down while the ionized fraction, 4.5 to 5.6 mg/dL, has not moved at all.
Why compensation is never overcorrection
The compensating system pushes the pH toward normal and stops. It aims at 7.40 and quits when it gets close. A pH that has crossed to the other side of 7.40 means a second primary problem, not enthusiasm. That single rule is how you name the primary disorder in a fully compensated gas: whichever side of 7.40 the pH sits on is the side the patient started from.

Watch outa normal pH with an abnormal PaCO₂ and an abnormal HCO₃⁻ is a fully compensated patient, not a well one. The disorder is still there. The defense is simply holding.
